It would be a good feature to have ability to add a header section where you can add the league logo and create a nice title for the forms.

Hi Michael,

I know this is a bit old, but I haven't really looked at these threads before.  You can easily do what you're proposing by adding an Acknowledgement section and leaving the Checkbox Message blank.  That basically gives you an empty section to play with, and using the HTML tab you can insert pretty much whatever you wish.  The below puts my "Blades_Logo.png" image on the left side of a nicely centered and not overly bolded title section.  Obviously you would adapt to your image and text.

<style>
    img {
    float: left;
    }
</style>
<img style="width: 95px; height: 86px;" alt="North Durham Blades" src="/public/images/common/Blades_Logo.png" />
<h1><center>North Durham Girls Hockey Association (A subsidiary of UMHA)</center></h1>
<h2><center><span style="text-decoration: underline;">2014/15 Girls only House League Hockey Registration Form</span></center></h2>
<span style="font-family: Calibri, Calibri; font-size: 16px;"> </span>

Hello Michael,

As Chris mentioned, this is already possible using the Acknowledgements feature of the online forms.

Marking as completed.
