Parents 

You will receive an email to sign up for a SportsHeadz account which will in turn allow you to use the Digital Health Check system. Click the link you receive in your email and sign up for your SportsHeadz account. Follow the prompts to verify. 

You should be re-directed to the SportsHeadz Signup page. Create your Sportsheadz account. If you have already done so with another child please sign in by following the link at the bottom.


You will then be directed to the Health Check Login Page. Login using these credentials you just created but please be aware that logins may be case sensitive. If you ever need to get back to the Health Check login page, it is always: health.sportsheadz.com


Once signed in, select the team you wish to complete a screening for. There will only be multiple teams if you have multiple children on different teams. Any events that require a screening will be listed. Simply click on the event to complete the screening.


Begin by selecting if you are attending or not. A health check is only required if you select attending. If you select Not attending, DO NOT ATTEND PRACTICE.



Answer all questions and hit SUBMIT


You can see your completed screenings once finished.
